Embodied AI Incident Catalogue

What happens when robots act in the physical world — documented, sourced, and severity-scored.

Software AI fails in text; embodied AI fails in physical space — a collision, a fall, a vehicle that halts in live traffic. This catalogue documents 117 such incidents involving physical AI systems, drawn from the AI Incident Database and our own research. Each is scored on the Embodied AI Incident Severity Index (EAISI) — a five-dimension, 0–20 scale built for cyber-physical harm rather than software bugs.

Waymo robotaxi drives into flooded road and is swept into creek; 3,791-vehicle voluntary NHTSA recall

Waymo autonomous driving system, 5th and 6th generation (Jaguar I-PACE platform) Waymo San Antonio, Texas, USA Near Miss
Details

On April 20, 2026, an unoccupied Waymo robotaxi operating in San Antonio, Texas, encountered an untraversable flooded section of road during heavy rainfall. Rather than stopping or rerouting, the system slowed but continued into the floodwater; the vehicle was subsequently swept into Salado Creek and had to be recovered days later. A second flood-related incident near McCullough Avenue and Contour Drive had occurred approximately two weeks earlier. Waymo paused San Antonio operations following the April 20 incident. On April 30, 2026, Waymo filed a voluntary software recall with NHTSA covering 3,791 vehicles equipped with 5th and 6th generation ADS across all operating cities (Phoenix, San Francisco, Los Angeles, Austin, San Antonio, and Atlanta). NHTSA announced the recall publicly on May 12, 2026. Waymo identified an area of improvement regarding untraversable flooded lanes specific to higher-speed roadways and is deploying an OTA software fix with refined extreme weather operations and restricted access to flash flood-prone areas. No passengers were on board at the time of either flood incident; no injuries were reported. This is Waymo's fifth recall since February 2024.

Avride delivery robot strikes and injures cyclist in Jersey City NJ, continues delivery route without stopping

Avride autonomous delivery robot (model unspecified; ~150 lb, Uber Eats deployment) Avride Jersey City, New Jersey, USA Physical Injury
Details

An Avride delivery robot (Uber Eats deployment) pulled into a Jersey City street directly into the path of cyclist Conor Shannon. Shannon was struck, knocked unconscious in mid-air, and sustained a broken shoulder and head injury. The robot did not stop at the scene; it continued its delivery route. Witness Sarah Ahern physically blocked the robot with her leg to document it before it continued. Shannon subsequently filed a lawsuit against Uber and Avride. Avride declined to comment, citing active litigation, but stated their fleet operates in accordance with traffic laws and safety regulations. The incident highlights a failure-to-yield error in the robot's intersection navigation logic and a post-collision behavioral gap: the system had no mechanism to detect collision with a human and halt. The incident adds to a growing Avride safety record: the company is simultaneously under NHTSA investigation for 16 robotaxi crashes in Texas (AV-010/EAISI-057) and had a prior pedestrian contact incident in Philadelphia (DEL-004/EAISI-056). Incident date reported approximately May 2026 (News12 NJ article published May 11, 2026); exact incident date not confirmed in available sources.

Cruise Robotaxi Initially Blamed for Ambulance Delay in Case Where Patient Later Died; Subsequent Reports Clear Cruise of Fault

Cruise Cruise Fatality
Details

In an initial report, a Cruise robotaxi was said to have delayed a San Francisco ambulance transporting Sammy Davis, a critically injured 69-year-old hit by a city bus. Davis later died. Subsequent clarification revealed that Cruise was not at fault for the fatality; the actual cause was a human-operated city bus. Despite this, the incident is included as it highlights challenges in the interaction between autonomous vehicles and emergency services in urban settings.

Avride autonomous robotaxi (Hyundai Ioniq 5 with Avride autonomous driving system); Uber ride-hailing partnership Avride Dallas and Austin, Texas, USA Physical Injury
Details

Between December 3, 2025 (Avride public launch in Dallas) and March 2026, Avride robotaxis were involved in 16 crashes across Dallas and Austin, Texas. One minor injury was recorded in December 2025 when a vehicle clipped an open pickup truck door; the victim did not require hospitalisation. Identified crash behaviours included: improper lane changes into adjacent vehicles, failure to slow or stop for slow-moving or stationary vehicles ahead, inadequate response to vehicles entering the lane, and striking stationary objects partially obstructing the lane. NHTSA characterised the vehicles as displaying excessive assertiveness and insufficient capability. All 16 crashes occurred with a safety monitor present in the driver seat; only one monitor intervention attempt was recorded. On May 8, 2026, NHTSA Office of Defects Investigation opened a formal investigation into Avride following preliminary video review of all 16 crashes. Avride stated it had implemented targeted technical and operational mitigations and that incident frequency relative to mileage had declined. Avride operates via an Uber app integration and had launched its Dallas robotaxi service December 3, 2025. Note: Avride also operates autonomous delivery robots in Philadelphia (see EAISI-056, DEL-004); this entry concerns the separate robotaxi platform.

Hugging Face LeRobot critical RCE vulnerability (CVE-2026-25874): unauthenticated pickle deserialization exposes physical robot actuator control paths

Hugging Face LeRobot open-source embodied AI and VLA framework, version 0.4.3 through 0.5.1 Hugging Face Global (open-source software; disclosed April 28 2026)
Details

On April 28, 2026, researchers publicly disclosed CVE-2026-25874 (CVSS 9.3 Critical), an unauthenticated remote code execution vulnerability in Hugging Face LeRobot (version ≤0.5.1). The flaw resides in the async inference PolicyServer component: pickle.loads() is used to deserialize data arriving over unauthenticated gRPC channels with no TLS. An attacker with network access can send a crafted pickle payload through SendPolicyInstructions, SendObservations, or GetActions gRPC calls to execute arbitrary code on the host. The PolicyServer sits directly in the physical robot control path—its outputs govern actuator commands on robotic joints—meaning successful exploitation could allow an attacker to issue arbitrary motor-control instructions to connected robots, posing potential physical harm to nearby personnel and equipment. The vulnerability was independently discovered by researcher 'chenpinji' in December 2025 and separately reported to Hugging Face; it was publicly disclosed April 28, 2026. The flaw remained unpatched at disclosure, with a fix planned for version 0.6.0. LeRobot's own team had previously created Safetensors specifically because pickle is unsafe for ML model serialization, yet the network-facing PolicyServer deserialized attacker-controlled input via pickle.loads() with a nosec comment suppressing the static analysis warning. LeRobot is used in academic and commercial robotics research contexts including manipulation arms, mobile platforms, and humanoid robot development.
